Registered Nurse- Home Based Primary Care
The Home Based Primary Care/ Geriatrics Extended Care Registered Nurse Case Manager will provide initial and continued assessment of nursing needs of the patient; teach the Veteran and caregiver effective and efficient ways to manage and deliver care in the home and monitor the Veterans' condition and the care provided in the home. Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
