Unit/Position Summary Remote Patient Management (RPM) is a patient care program utilizing two way audio-video coupled with remote patient monitoring devices to help care for patients in their homes. It is staffed with experienced care managers and allied health professionals who utilize subjective and objective information received from in home telehealth systems to provide proactive and holistic care to patients. The RPM care model alerts our clinical staff to subtle deterioration in a patient's condition, allowing them to perform early interventions in order to avoid adverse outcomes. The RPM
