Overview
Atlantic Health System is seeking an experienced Nurse Practitioner for an exciting new initiative - Primary Care Home Care Program part of Atlantic Medical Group. The Nurse Practitioner will visit patients who are 65 years or older in their homes for primary care visits. The territory is Morris County (approx. 5 calls per day, set up in local silos each day) and will also have some in-office visits and tele visits as well. The successful candidate will have a passion for this patient population, one to two years exp. and an eye on quality improvement and community initiatives a strong plus.
Responsibilities
Job Functions:
Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.

Candidate must have excellent communication and computer skills.
Qualifications
Education:
Masters degree required
License/Certifications:
ACLS required
BLS required
Valid DEA/CDS and NP license required
Relevant Experience:
2 years previous NP experience preferred
Experience with wound care, incision drainage, cath. change, and primary care preferred
Coordination of patients
Ordering/Coordinating testing
Communication with referring cardiology/internists or decisions re: patients and follow-up appointments admission and discharge
Educated/trained to provide health promotion and maintenance through diagnosis and treatment of chronic and acute conditions
Conduct physical exams; interpret medical history; work closely with team
Experience with Palliative Care or Geriatric patients a plus
